Request for Proposal (“RFP”) forms may be obtained by email from carl@4mativ.org on or after 1:00 PM, Central Standard Time (CST), Friday, April 18th, 2025. Proposals shall be submitted via email in PDF form to carl@4mativ.org and shall include the subject line: “Response to 4MATIV RFP for Hogan Preparatory Academy SY25-26 – [OFFEROR NAME]”

Proposals will be received at the above email address until 11:59 PM, May 9th, 2025. Proposals received after this time will be rejected. Faxed or hard copy mailed proposals will be rejected and not considered.

FSMC RFP 2025-2026

This solicitation is for the purpose of entering into a contract for the operation of a food service program between the school/ district hereinafter referred to as the Local Education Agency (LEA) and the Food Service Management Company (FSMC). Each bidder must submit a complete response to this solicitation using the forms provided. No other documents submitted with the Request for Proposal (RFP)/contract will affect the contract provisions. If the FSMC provides additional provisions they must be included on the RFP/contracts additional provisions page and they must be reviewed and approved by the State Agency.

All proposals must be received by: 1:00 PM CST, April 18. 2025. Proposals will not be accepted after the deadline.
